How to register with the Social Security Board?
* Fill out a Social Security Board Employee's Registration Form.
* Submit the form to the Social Security Office, with proof of birth (Certificate of birth, passport, ect.)
How to claim for Sickness Benefit?, * Have your doctor complete the Medical Certificate.
* Complete the Sickness Benefit Claim Form which is on the reverse side of the Medical Certificate.
* Submit the form to the Social Security Office.
How to claim for Maternity Benefit? Have your doctor complete the Medical Certificate of Expected Confinement once the woman stops working, and the Certificate of confinement once the baby is born. Complete the reverse side of the form.
Complete the Maternity Expense Form (necessary only if the birth took place at Peebles Hospital).
Submit the forms to the Social Security Office.
